Highway Contractors Receive 2019 Statewide Safety Awards

(INDIANAPOLIS) As part of its ongoing effort to lead the Indiana highway, heavy and utility construction industry in the area of workplace safety, Indiana Constructors, Inc. (ICI) presented its 2019 Safety Program Awards at the association’s annual meeting on Nov. 21 in Indianapolis.

An independent panel of judges chose the following firms for their overall excellence in safety culture as shown through metrics, innovative programs and anonymous supervisor and onsite worker evaluations.

2019 Safety Program Awards

ELIGIBILITY

To be considered for an award, applicants must meet the following eligibility requirements:

  • Current ICI membership;
  • No OSHA citable work-related fatalities since January 1, 2018;
  • Must have one or more active jobsites available for visit during August and September 2019;
  • Must allow a random and unannounced jobsite validation and worker interviews;
  • Award recipients must work with ICI’s Safety Committee to participate in a safety roundtable gathering in early 2020.

Awards will be announced at ICI’s Annual Membership Meeting on Thursday, November 21.

COMPLETING THE APPLICATION

The ICI application process consists of three parts:

  1. Interactive application form which is filled in and submitted electronically once you have provided all of the requested information. Each section of the application clearly states what is required for that section.
  2. The application form will also request some independent documentation. Please save these documents as a PDF file to upload with the application.
  3. Once you complete the application form, ICI will send you links to electronic supervisor and onsite worker surveys. You will need to provide those links via text or email to your supervisors and onsite workers so they can take the one/two-minute survey.

DEADLINE

Applications are submitted electronically and must be received by Friday, June 28, 2019. At least the minimum number of all supervisor and onsite worker surveys must be submitted by the June 28 deadline (refer to application for minimum number needed).

CONFIDENTIALITY

To ensure fairness in the judging process, ICI will remove all identifying information, such as company name and address, from the applications and supporting documentation before providing information to ICI’s Safety Program Awards Selection Committee.

COMPANY SAFETY PROGRAM

Please provide “yes” or “no” answers to three questions, adding explanations and/or comments as appropriate.

  • Please attach a sample copy of one of your jobsite safety plan, hazard analysis and preventative action.
  • Please attach a sample copy of your written plan to provide task-specific, hands-on training for new hires and employees who transfer to a new task.

SAFETY PROGRAM COMPONENTS

Please attach a copy of the Table of Contents from your company’s safety manual.

TARGETED QUESTIONS – SHORT ANSWER

Please provide brief answers to the two questions. These short answers provide additional insight into a contractors safety efforts that safety numbers may not show; so your written answers to these questions will help judges to better understand a contractor’s safety efforts and programs. Although ICI will remove all identifying information, please refrain from using the company name and/or company-specific acronyms in this section.

SURVEYS

ICI uses electronic surveys to make it easier for companies to distribute and request anonymous feedback from employees about the company’s safety program. Contractors without the capabilities of doing the surveys electronically should contact Ashley Aiken at aaiken@indianaconstructors.org at (317) 634-7547 for paper surveys and instructions.

APPLICATION SUBMITTAL

Completed applications will be automatically sent to ICI via the interactive application form. Survey forms should be submitted by Friday, June 28, 2019.

Form FHWA-1273

Federal Highway Administration (FHWA) recently completed a review of a sample number of subcontracts executed for performance on federally funded Indiana Department of Transportation (INDOT) highway contracts. FHWA found that a significant number of contracts did not contain form FHWA-1273, Required Contract Provisions, Federal-Aid Construction Contracts. This form must be physically incorporated into subcontract agreements as required per federal regulation 23 CFR 633.102 (e). Section (e) language is shown below:

(e) The contractor shall insert in each subcontract, except as excluded by law or regulation, the required contract provisions contained in Form FHWA-1273 and further require their inclusion in any lower tier subcontract that may in turn be made. The required contract provisions of Form FHWA-1273 shall not be incorporated by reference in any case. The prime contractor shall be responsible for compliance by any subcontractor or lower tier subcontractor with the requirements contained in the provisions of Form FHWA-1273.

Prime contractors may insert the language of form FHWA-1273 into the body of their subcontract document or include a statement requiring compliance to the form contents with a reference to a subcontract attachment. Prime contractors may not reference the form only or reference an external link to the form.

Davis Bacon Statewide Residential Wage Survey Info

Federal Davis Bacon is requesting wage and project data submissions for any/all public and private work performed from Aug. 1, 2017 through July 31, 2018 on, or incidental to, residential projects (defined as) for the construction, alteration, or repair of single family houses or apartment buildings of no more than four (4) stories in height. This includes all incidental items such as site work, parking areas, utilities, streets and sidewalks.

The results of this survey will be used for setting Indiana residential wage settings for the next 10 years, which means all HUD and other housing, apartment, dorm construction and incidental utilities.

The Davis Bacon requested wage and project information can be submitted electronically or by mail.

Many Indiana contractors should have received this background letter from the Indiana Laborers.

The Laborers tell us that ABC of Indiana/Kentucky and other associations are attempting to get as much information submitted as possible, so they can get ABC wage rates set on qualifying projects.

Note: The Davis-Bacon and related Acts require the Secretary of Labor to determine the prevailing wage rates for corresponding classes of laborers and mechanics on projects in the area which are of a “character similar” to the proposed contract work to which the determination will be applied.
